2. TERMS
a) MATINGS ~ Pilot and _________were bred on _______/_______,2011
If natural cover breeding is selected, costs for transportation of the bitch are the responsibility of the bitch owner.
If artificial insemination is selected, the bitch owner is responsible for all expenses relating to collection, preparation and shipping
the chilled semen as well as expenses for the bitch being inseminated. If no puppies result from this breeding, there is no
reimbursement of fees. A minimum of two days advance notice needs to be provided to stud owner in order to make
arrangements for the stud dog to be collected. A credit card number is required to be supplied to the Veterinarian to collect,
prepare and ship fresh semen to the bitch owner's Veterinarian.
b) STUD FEE ~ ______agrees to pay to Rayma the sum of $_____ or pick puppy, upon a pre agreement of the bitch owner,
following the live birth of two or more healthy puppies as a result of this breeding. If the breeding does not result in the birth of
two or more healthy puppies, ______will not owe the stud fee to Rayma and this contract will become null and void.
c)EVALUATION ~ ______agrees to provide to Rayma photographs of all puppies at 2-3 days of age and 7-8 weeks of age.
d) FOLLOW-UP ~ ______agrees to notify Rayma of any health or temperament information that she receives regarding the puppies
of this litter from future owner of the puppies.
3. WARRANTIES
a)______agrees to provide on going veterinary care and vaccination at her expense for any puppies sired by Pilot for as long as said
puppies remain in her possession.
b)______agrees to provide adequate socialization, training and compassionate care to the puppies to ensure for well adjusted
companion animals. Further, _____agrees to place all puppies in homes that have been properly screened for the benefit of the
puppies, and not to place the puppies in new homes prior to the age of 8 weeks.
c)Both Rayma and _____ guarantees that their dogs were in good health at time of breeding, free of know contagious diseases and
have received vaccinations typically required for their ages at the time of breeding. A Brucellosis test is also required on both Pilot
and ____before being bred.
4. ATTACHMENTS
Copies of the following documents for each dog.
a) A complete pedigree including registration number of at least three generations
b) The dog's most recent eye exam results or CERF certification
c) The dog's hip exam results, OFA or Penn Hip. Must be Fair or better.
d)Any copies of health or genetic screening performed on the bitch.
5. AGREEMENTS
a) The bitch owner would read, agree to and sign the MASCA Breeders Code Of Ethics and any direct offspring of this stud dog
would only be registered as Australian Shepherds of the Miniature variety.
By signing this agreement, Rayma and ______certify that the information herein and that all attachments to this contract are true
and complete to the best of their knowledge. The entire agreement between the parties is contained herein unless there is a written
addendum to this agreement signed by both parties.
